Daily Physical Activity and Healthy Living Plan
- Complete all of the attached questions to complete your Personal Healthy Living Plan. Complete all four sections of the plan. Please complete the assignment on a word processor/computer.
- Once you complete all four sections, put it together in a single package and staple it together or put it into a duotang. Ensure that your name and student number are clearly marked and it is labelled Healthy Living Plan.
- Hand in your completed and labelled package to the Main Office in the box labelled Grad Transitions IN box. Once it is marked, it will be placed in the OUT box. Make sure you pick it up, as this is your evidence of completion of this aspect. Check the updated spreadsheets to ensure your completion has been recorded.
